Dear Prospective Burleigh Kindergarten Parent,

The Burleigh Elementary School Community is fortunate to have a 37 acre site and a top notch school building that includes a newly remodeled six-room kindergarten suite, a huge library/media center, multiple art rooms, 3 computer labs, SMARTBoards in every classroom, a Discovery/Science Lab, a large dedicated orchestra/band room, a dedicated vocal music room, two full-sized gymnasiums used exclusively for physical education, a large cafeteria used for our student lunch program, an on-site nature center, tennis courts and an outdoor play area that students refer to as the "Great America" of playgrounds.

We cordially invite you to a Parent Information Night (PIN) for prospective kindergarten parents on Tuesday, January 12, 2010 at 6:30pm. You will be able to meet with Burleigh staff, which we hope will help you better understand our kindergarten program. The Parent Teacher Organization and our Burleigh Parent Liaison will be available to give tours after our PIN program. The Burleigh PTO and Parent Network organization provides new-to-school parents the opportunities to meet other parents and get involved in your child's education at Burleigh. We encourage you to make an informed decision regarding your child's future education. Dear Parents:
Do you have a child who will be 5 years old by September 1st, 2010, who you might be considering enrolling into Burleigh Elementary's Kindergarten program next fall? All parents of children who will be attending either our half-day or full-day kindergarten program must register their child by January 29th. You will need to complete a Student Enrollment and Information Form and Kindergarten Preference and Transportation Sheet. We just wanted to remind you of a few items we will need in order to complete the preliminary enrollment procedure. Upon turning in the Student Enrollment and Information Form you will need to have with you:
- A certified copy of your child's Birth Certificate (we will not be able to accept photocopies that are not notarized)
- Proof of Residency (Utility bill showing current address, current lease agreement…)
- Sign bottom portion of form
- Home Language Survey
Please make sure that each section of the form is filled out completely.
You will also need to complete a Student Immunization Record, Health Exam Information Form, Health History, and Eye Health Examination which should be completed and returned when your child comes in for Orientation in May 2010. We do realize that many students will be seeing their pediatrician after this date and having additional immunization – this information can be called into our health room at 262-781-5599 and we will update the immunization record verbally at that time.
For your convenience you may drop off completed enrollment forms at our evening Parent Information Night (PIN) on Tuesday, January 12 at 6:30pm and/or register your child anytime during our regular business hours (8am to 4pm, Monday through Friday) from December 8 to January 29.
