EMERGENCY NURSING SERVICES
The School District of Elmbrook shall provide emergency nursing service to handle the care of students experiencing accidental injuries, illness and requiring medication. Emergency nursing services shall be provided under the direction of a nurse registered in Wisconsin. The District Health Advisory Team shall serve as a resource for emergency nursing services policies and procedures. Emergency nursing services shall be available during the regular school day, including curricular and co-curricular activities of students. Although the school nurse is not required to be present at all school sponsored activities, students having indication of illness or injury shall be referred to the authorized person in charge when first aid is needed. An annual review shall be made of the emergency nursing services and a report of this review shall be submitted to the School Board.

SCHOOL NURSING SERVICES
The School Nurse is available to daily assist students, family and staff in understanding and integrating health issues. Other responsibilities of the nurse encompass wellness promotion for staff and students, communicable disease control, health assessment and counseling, and emergency first aid services. An individual health record is maintained on each student. Please keep the nurse informed regarding current health information while your child is attending school.
Health Room Assistants, under the supervision of the School Nurse, provide emergency first aid services during school hours. Several of the district schools also have parent volunteers who are trained in first aid through The American Red Cross who assist both the School Nurse and/or the Health Room Assistant in the health room during busy times.

SCHOOL HEALTH POLICIES
Emergency Card
If your child becomes ill or injured at school and is unable to return to class, he or she must be sent home. It is very important to inform the school of a work and home phone number where you can be reached during school hours. Please fill out and hand in your child's STUDENT EMERGENCY CARD as soon as possible. Please designate two (2) reliable alternates to contact if you cannot be reached. A well child belongs in school, and an ill child belongs at home.

Health Fitness Care - High School Sports
Elmbrook High School's LICENSED ATHLETIC TRAINERS provide services of INJURY PREVENTION, EVALUATION, MANAGEMENT, and REHABILITIATION. An athletic trainer is available for all sport practices and home contests. The trainer is available by cell phone for consultation. Also, several local physicians volunteer their time by attending events of higher injury risks. The schools' well equipped training room allows the trainers to provide rehabilitation services as prescribed by a physician.

Field Trips and After School Activities
Students having indication of illness or injury while attending a field trip or after school activity shall be referred to the authorized person in charge who shall administer first aid where needed. In cases of serious illness or injury within the district, the student will be transported by ambulance/police vehicle to Elmbrook Memorial Hospital. When an injury or illness occurs outside of the district, the student shall be transported to the nearest emergency facility. The parent/guardian, when available, or person(s) designated on the Student Emergency Information Card shall be notified as soon as practicable.

Student Chemical Abuse
Parents shall be notified when a student is suspected of being under the influence of chemicals. If the parent/guardian cannot be reached, alternate person(s) designated on the Student Emergency Information Card shall be notified in order to remove the student from school. If the student seems to be suffering acutely, he/she will be transported by ambulance to Elmbrook Memorial Hospital.

School Health Policy Regarding Medication or Treatments
When a child needs prescribed oral or injectable medication administered by the health room during school hours, it is necessary to have WRITTEN PERMISSION FROM A PHYSICIAN on file with the School Nurse. This written permission must state the amount of medication, dosage, and time it is to be administered. FOR NON-PRESCRIPTION DRUGS, WRITTEN PERMISSION FROM THE PARENT must be on file. Please include the name, amount, and the time the medication is to be given. All medication must be in its original container and labeled properly. Medication will not be sent home by the school on the bus. High school students may keep their own medication in properly labeled containers.

Physical and Dental Examinations
All students entering any of the district's schools are encouraged to have a school health examination by a licensed physician completing the district's prescribed form. Those requirements may be waived for health or religious reasons. Biannual dental examinations are also encouraged.

Required Immunizations for Entering School
State law requires all public and private school students to present written evidence of immunization against certain diseases within thirty (30) days of admission. These requirements can be waived only if a properly signed health, religious or personal conviction waiver is filed with the school.

Hearing/Vision Screening
Hearing and/or vision screening on students is done upon request of parent or teacher.
