GUIDELINES FOR MEDICATION ADMINISTRATION
State Law and School Board Policy specify definite guidelines for the handling and dispensing of medication for students while in school. State Law (Statute 118.29) has established separate procedures for medication prescribed by a physician and non-prescription medications (over-the-counter). Physicians in the area are aware of the Law and the Policy.
PRESCRIBED MEDICATION: Prior to administering prescribed medications, the school must be provided with a signed, written authorization from the parent and signed, written instructions from the prescribing physician.
The physician's statement must include:
1. the prescribed drug and dosage
2. the frequency of administration
3. the condition and circumstances requiring the administration of the medicine
NON-PRESCRIPTION: Non-prescription medication can only be administered by school personnel with written parental approval and provision of the medication. Written instructions must be signed by the parent and shall include:
1. the prescribed dosage
2. the frequency of administration
3. the conditions and circumstances requiring the administration
STUDENTS MAY SELF-ADMINISTER AT THE HIGH SCHOOL LEVEL ONLY: Forms for both physician and parent authorization can be obtained from the Health Room.
Parents are responsible for the delivery of prescribed and non-prescription medication to and from the Health Room at the elementary and middle school levels and for those high school students for whom self-administration is not deemed to be appropriate.
MEDICATION CONTAINERS: Medications to be administered at school must have the following information printed clearly on the original container:
1. child's full name and grade
2. name and dosage of the drug
3. frequency of administration
4. physician's name (prescribed meds only)
UNUSED MEDICATION: Unused medication will not be returned to any student. Parents must come to the Health Room for unused medication or it will be disposed of.
ALL CONSENT FORMS MUST BE RENEWED EACH SCHOOL YEAR AND/OR ANYTIME A MEDICATION IS CHANGED.
