Registration Instructions
To enroll your child at any grade level:
1. Please complete the Student Enrollment Form.
2. Please bring the Student Enrollment Form to the Pilgrim Park Middle School office along with proof of identification and residency:
- An original or certified copy of your child’s birth certificate (no un-notarized copies). Birth certification will be returned to you upon verification.
- Resident home owners can bring a closing statement, mortgage statement or tax bill AND a recent utility bill; Leasers/renters may bring a current lease/rental agreement that includes parent/guardian names, manager or owner name and telephone number, lease start date, lease termination date, and cancelled check showing payment or rent for a current period.
Any additional forms needing completion will be distributed to you when you hand in the Student Enrollment Form and proof of identification/residency. We look forward to meeting you!
