Kindergarten Registration Instructions
For Fall 2012 Kindergarten-eligible students (those who will be 5 years old on or before Sept. 1, 2012):
1. Please complete the Student Enrollment Form.
2. Please complete the Kindergarten Options Form.
3. Please complete the Home Language Survey.
4. Please bring these three forms to the Tonawanda Elementary School office along with proof of identification/residency:
- An original or certified copy of your child’s birth certificate (no un-notarized copies). Birth certification will be returned to you upon verification.
- Proof of Residency: Resident home owners can bring a closing statement, mortgage statement or tax bill AND a recent utility bill; Leasers/renters may bring a current lease/rental agreement that includes parent/guyardian names, manager or owner name and telephone number, lease start date, lease termination date, and cancelled check showing payment or rent for a current period.
Additional forms will be sent to you once you have registered your child for Kindergarten. Please register by mid-February. We look forward to meeting you!
Registration Instructions - All Grades Except Fall 2012 Kindergarten
To enroll your child at any grade level with the exception of 2012 Kindergarten (see above for Kindergarten Registration information):
