Complete Physical Exam

Welcome to the Complete Physical Exam!

This process will replace our Annual HRA Biometric Screening event for the Insurance Wellness Incentive. We have partnered with our Elmbrook Family Wellness Center to provide employees/spouses a 60-minute annual physical exam. The date range for the Wellness Year begins 11/1/25 and the required visits to meet the incentive program must be completed by 10/31/26. Employees who choose to participate in the Wellness Program will receive an incentive of monthly premium savings (monthly $60/single, $60/spouse, $120/family). Effective January 1, 2026.

Want to continue to see your current provider for your annual physical?

You are required to bring the following forms with you to the Doctor's office to have your Doctor complete. Once you have completed your physical exam with your Doctor, you are required to call the clinic to schedule an appointment to bring in the completed physician form (below) and to schedule your lab work at the clinic (where labs are free).
