Eligible teachers who have submitted their Intent to Level Up in Skyward are now able to complete the Level Up Application. The Level Up Application is completed in Skyward, and will be made available from February 1st, 2019 through April 1st, 2019.

The deadline to file your 2018 Flexible Spending Account (FSA) Dependent Care and Limited Purpose claims with Optum Bank is March 31, 2019. The same deadline holds true for members filing 2018 Health Reimbursement Arrangement (HRA) or Medical FSA claims with Diversified Benefit Services (DBS). Note, there is no deadline to seek payment through a Health Savings Account (HSA).

Have you scheduled your Health Risk Assessment Results visit? Don't wait--time slots are filling up--be sure to schedule as soon as possible in order to meet the June 28th deadline. In addition to weekday appointments, two Saturday dates have been added for HRA visits!
Plan to attend the District-wide "Financial Wellness Fair" scheduled for Monday, February 25th from 3:30 to 5:30 PM at the Burleigh Elementary Cafe.
As a courtesy to others, if a patient is more than ten minutes late to an appointment at the Elmbrook Schools Wellness Center, it may be necessary to reschedule the visit. Depending on the schedule of the day, Wellness Center staff will try their best to accommodate the needs.
