IMPORTANT CHANGES--Health Risk Assessment & Biometric Screening have NEW REQUIREMENTS for co-pay reduction incentive: 

  1. Health plan-covered spouses must participate.
  2. Biometrics must be performed by Healthstat.
  3. Questionnaire & Biometrics due by 10/25/18.
  4. Participant HRA results visit by 3/29/19.
  5. Shoo the Flu clinics at Biometric events.
