Health Risk Assessment (HRA)

In order to qualify for the premium incentive ($60 per month for employee, $60 per month for spouse), completion of the questionnaire, biometric screening and if specified, follow-up visits, are required for all employees and spouses on the health plan. If you will not have health insurance coverage with Elmbrook Schools as of January 1, 2022, it is not necessary to complete the HRA requirement. 

Please look at this flyer for steps and information on completing the questionnaire and scheduling your biometric screening appointment. The sign-up deadline is October 10, 2021. If you do not need to be available for students at the start of the school day, please schedule your appointment for 8:45am or later. All events this year will be held at Elmbrook’s Hillside location (2250 N. Lynette Lane Brookfield, WI 53045) from October 11th through October 16th and October 18th through October 23rd. Masks will be required to be worn. Members who live or travel outside the area should contact Beth McCabe at mccabee@elmbrookschools.org for alternate instructions. 

You will receive a confidential results packet mailed directly to your address from Everside. This packet will provide a comparison with the prior year’s results and indicate a risk factor. Follow-up visit(s) by June 30, 2022 are required to maintain the incentivized premium as indicated in the chart below. In-person or virtual follow-up visits can be scheduled through the Wellness Center at 262-214-1101.

A Shoo the Flu Clinic will be offered at Hillside for HRA participants only. A consent form will need to be completed prior to arrival.  All health plan members are offered flu vaccines at no cost. If not participating in the HRA event, call the Wellness Center at 262-214-1101 to schedule an appointment to receive your flu vaccine.

Shoo the Flu Clinics
The Wellness Center will be hosting four Shoo the Flu Clinics in September and October. These clinics are available to all employees. It is also available to all family members on the health plan. The clinics will take place on September 29th, October 6th, October 13th and October 20th from 4:30pm – 6:30pm. Call the Wellness Center at 262-214-1101 to schedule your appointment. Please complete and bring this consent form to your appointment.
