The 1095-C form for tax year 2021 will be mailed out this week to those who requested paper copies. Those individuals who requested electronic copies, as well as those that elected paper copies, will be able to access their forms through Skyward Employee Access this week as well. To access the form on Skyward, go to Employee Access > Employee Information > Payroll > 1095 Forms.

The 1095-C form contains medical insurance enrollment data based on requirements from the Affordable Care Act. The 1095-C is NOT needed to file your individual tax returns but the IRS recommends you keep the form with your tax files.
