2026 CPE Wellness Incentive Program:

What is it:  An annual complete physical exam for employees and spouses.  This incentive program is focused on preventive screenings and a complete physical exam.  This program is voluntary and is available to employees and spouses who are enrolled in the Elmbrook health insurance plan.

Why would I participate:  To ensure you are getting care via your doctor annually and to understand the importance of annual preventive screenings.  If you and your spouse participate you will receive monthly premium savings $60/employee, $60 spouse.

Timeframe:  11/1/25 through 10/31/26.   You must complete your CPE at our Elmbrook Family Wellness Center or with your own provider no later than 10/31/26.  This deadline will not be extended so please DO NOT wait until the last minute to schedule your CPE.  You may not get an appointment in time!

If you visit your own provider:  If you choose to have your Complete Physical Exam (CPE)* with a community provider, your provider will need to attest that you have performed your annual CPE within the wellness year (11/1/2025-10/31/2026). The community provider form will need to be returned to the Elmbrook Schools Family Wellness Center, not the Elmbrook School District, during your CPE health review appointment. If your appointment is performed virtually, forms must be submitted prior to your appointment. This form will be documented in your medical record, and it gives consent for health center staff to review your medical information during the health review. This form alone does not meet incentive requirements.  You must schedule a CPE review at our Elmbrook Family Wellness Center no later than 10/31/26.  Please call 262-214-1101 to schedule this appointment.

Lab Services:  Most labs are done at no cost at our Elmbrook Family Wellness Center.  This may vary depending on the type of labs requested.
