If there is one thing that I can stress to you about your benefits it would be this! 

WRS death benefits are always paid according to the most recent, valid beneficiary designation on file with ETF prior to your death. Your beneficiary information does not automatically change when a life event occurs, such as a marriage or a divorce. Review your beneficiaries and your needs regularly. Birth, adoption, death, marriage and divorce are all common life situations that can change how you want your beneficiary designation structured.

Note: The date the form is signed is not the date it becomes effective. A Beneficiary Designation form does not become effective until received and approved by ETF. The person filing the designation must still be alive when ETF receives the form. An acknowledgment will be sent when this designation has been reviewed and accepted. Invalid designations will be rejected. ETF's acceptance of your designation does not guarantee that your account will provide a benefit upon your death. Contact ETF with questions about a potential death benefit from your account.

Submit the form to ETF. Make a photocopy of the completed form and keep for your records.
