No cost flu vaccines are available to ALL Elmbrook employees and health plan family members. Vaccines are now available with an appointment. Shoo the Flu clinics are offered at the Wellness Center:

IMPORTANT things to note:
- Flu shots are by appointment only. Call the Wellness Center at 262.214.1101 to schedule an appointment.
- Print, complete and bring this pre-filled form with you: Flu Immunization Participant Informed Consent Form
- Employees not covered under the Elmbrook health plan:
- Bring a picture ID, (e.g., State ID, Drivers License, District ID)
- Be able show proof of recent active employment (e.g., payroll deposit, recent pay advice print or screen shot, access to active Skyward account through app).
- Employees and spouses covered under the health plan may receive a flu vaccine while attending a Health Risk Assessment event.
- For dependent children of employees covered under the health plan, schedule an appointment at the Wellness Center. Non-HRA participants will not be allowed to enter nor are offered flu shots at the HRA events.
- If unable to attend any of the Wednesday clinics, contact the Wellness Center for an appointment time.
