Tax forms 1095C were delivered this week to staff who were offered health insurance benefits in calendar tax year 2017. Take look to learn more about this IRS-required form:

• 1095C's are NOT needed to complete 2017 income tax returns.
• If you didn't receive your form through interoffice mail, watch for it in your home mailbox.
• The minimal monthly cost of $98.70 listed is established for Elmbrook's plan by the IRS Affordable Care Act rules. This is NOT what staff paid for health coverage. Here are the 2017 monthly premium rate co-pays included on your W-2:
  • HSA: Single $48.70; Family $117.42; Family Buy-Up $234.84
  • HRA: Single $67.80; Family $163.50; Family Buy-Up $327.00
• 1095C's will become available on the Skyward Staff Portal shortly.
