Reminder! The deadline for the Health Risk Assessment follow-up requirement is approaching. The required follow-up visit, for employees and spouses, to maintain your reduced wellness premium on health insurance ($60 per month per participant/$120 per month for family coverage) needs to be completed as follows:

   -New enrollments and participants never having a follow-up visit: one visit scheduled by June 30, 2021
   -Those with one Risk Factor: one visit scheduled by June 30, 2021
   -Those with two or more Risk Factors: the second of two visits scheduled by June 30, 2021
