Complete your required follow-up visit(s) to maintain your reduced wellness premium co-pay ($60 per month per participant, $120 for family coverage).

Reminder mailers from healthstat were recently sent to home mailboxes of participants, employees and spouses, with two or more risks who still need a first follow-up visit.  Note, the first follow-up appointment date was extended to no later than April 30, 2021. 

Participants with one risk facto or new to the plan requiring one visit and those with two or more needing a second visit, are required to have this completed by June 30, 2021.

• Wellness Center Follow-Up visits are required:
  • New enrollments and participants never having a follow-up visit by June 30, 2021;
  • With 1 Risk Factor, 1 visit between December 1, 2020 and June 30, 2021;
  • With 2 or more Risk Factors, 2 visits, 3 months apart: December 1, 2020 March April 30, 2021 and April 1-June 30, 2021.
