The "Shoo the Flu" clinics are back for ALL employees and health plan-covered spouses and dependents (ages 3+).  NO cost flu vaccinations, paid by Elmbrook Schools, are offered at all Health Risk Assessment Biometric Screening Events on the following dates. Flu shots are also available at the Family Wellness Center by scheduling an appointment at 866-959-9355.

Print, sign and bring along with you the Patient Informed Consent Form for speedy access to the immunization.
