As required of employers with self-funded health insurance plans, tax form 1095C's were distributed to all who were offered health insurance during 2018.  Per IRS rules, this form is not necessary to file individual income taxes but rather a reporting requirement for employers.  It is recommended that this form is to be kept with personal individual income tax files.

To note, the amount of $97.30 published on the 1095C form per IRS guidelines is the single non-wellness monthly premium which is not the monthly single premium paid by most employees.  The monthly co-payment for Wellness participants is $47.30-Single and $113.98-Family.  (The wellness incentive for Health Risk Assessment completion includes a discount of $50-Single and $120-Family.)
