Mark your calendars for the annual Health Risk Assessment (HRA) Biometric Screening & Shoo the Flu Events in your building.  Online questionnaire and appointment scheduling will be available beginning September 2nd thru October 11th ONLY.  Participants are welcome to schedule appointments at any District event. Note the HRA updates and reminders for 2019-20:

• 2020 Health plan members (employee & spouse) required to participate for premium co-pay incentive. (Dependent children not included.)
• Online survey questionnaire completion and biometric appointment scheduling required. Access available 9/2-10/11/19 ONLY.  (Instructions)
• Biometric Screenings will only be allowed through Building Events - No longer available at the Wellness Center.
• New enrollment in the Health Plan as of January 1, 2020?  Email Lori Golomski, Sr. HR Specialist at golomskL@elmbrookschools.org to select an appointment time.  You/spouse will complete your survey online at your Events appointment.
• Spouses (& Retirees) living/travelling 100 or more miles outside the Elmbrook area during the Event dates may obtain a remote questionnaire packet with biometrics provided through an outside provider--Wellness Center if considered Primary Care Provider.  Participant responsible to assure provider submits biometric results.  Postmark deadline is October 28, 2019.  For details, contact Lori Golomski at golomskL@elmbrookschools.org.
• HRA Results Visits Required at Family Wellness Center by June 30, 2020:
  • Participants who have never had an HRA Results Visit;
  • Participants with one or more identified risk factors.
• Monthly Premium Co-Pay Incentive:  $60-Single, $60-Employee, $60-Spouse/$120-Family.
• ALL employees and health plan covered family members (ages 3+) are eligible for no cost flu vaccines - Walk-in at Event below or through Wellness Center by appointment--schedule at 262.214.1101.

Note, if you do not need to be in a classroom with students, please schedule your appointment after school begins to allow fellow staff members the early appointment times to complete their screening prior to student arrival.  Thank you.
