Elmbrook Transition Center

The Elmbrook Transition Center is an adult transition service for 18-21 year-old young adults in the School District of Elmbrook. Aligned with the guidance provided by the Department of Instruction, the Elmbrook Transition Center focuses on programming for students who are of typical graduation age but who continue special education services for a period of time. Together we partner with families to focus on transitional skills and priorities to further develop independent living skills, social communication, vocational abilities, and/or recreational activities. The Elmbrook Transition Center focuses on community partnerships with many local transportation agencies, community businesses, volunteer partners, and the Division of Vocational Rehabilitation.
